How do I fill out DD Form 2808?
The report consists of three pages preceded by brief privacy act statement. In total there are 86 fillable fields that must be completed with the appropriate medical information including results of the clinical evaluation of all organs and organ systems, laboratory findings, test results, physical and psychological profile. At the end of the report there is a field for diagnosis, recommendations and final decision.
